If your organization has thought about participating in the AHCA/NCAL National Quality Awards Program, this is your opportunity to complete the Bronze Award application from beginning to end with coaching support along the way.
This workshop is about more than an award; besides gaining a better understanding of what examiners are looking for and how to use the award criteria as an improvement tool, you will learn how to use the award process to focus, align, and accelerate your performance excellence efforts – becoming a high-performing organization.

What To Bring
Please note that all information you bring is for your use only; it will not be shared or given to others at the workshop. These items are necessary to complete the application.
- A laptop computer.
- 2025 Bronze Quality Award Application which you can download from the AHCA Quality Awards website after July 9, 2024.
- A copy of your mission statement (and your vision statement if you have one).
- A list of your major employee groups, with the number of desired employees for each group
- An organizational chart of your company If your facility is owned by a parent company, include any links/reporting structure to it.
- A basic geographical description of your local market area, defined as the area from which you draw residents or other key customers and/or staff members, and a list of competitors.
- The names of the data sources that you currently use to access comparative and competitive data.
- A list of the steps of your process to fix problems or make improvement
- Identify one thing that you have improved or fixed using the steps you listed ab Include a numerical measure of the improvement. If you are a SNF, this should be clinical in nature.
- If you are an assisted living provider, identify a second thing that you have improved or fixed using the steps listed above.
