2020 IHCA Foundation Juckette Family Veteran Scholarship

Scholarship criteria and information
Applicants for the IHCA Foundation Juckette Family Veteran Scholarship, made possible by the Linda Juckette Family Gift Fund, must meet all of the criteria listed below in order to qualify.

• Applicant must be currently serving or have served in the U.S. military, National Guard or Reserves. This scholarship is specific to honorably discharged veterans, active duty and Guard/Reserves.
• Applicants must be enrolled or accepted in a CNA, LPN, RN or CDM (Certified Dietary Manager) program in Iowa.
• Scholarships are for the 2020-21 academic year; therefore, applicants must be taking classes during the fall 2020 and/or spring 2021 semesters. This is a one-year scholarship that covers the cost of the course/program up to $2,000.
• Submit a complete application form, including a 300-word essay, acceptance letter or transcript, one letter of recommendation and copy of veteran’s record of service demonstrating an honorable discharge or letter from commanding officer certifying active duty status or copy of duty orders.

UPDATE: Complete applications with all requirements must be submitted by JUNE 5, 2020.

• The IHCA Foundation Board of Directors will judge qualified applications.
• Recipients will be notified of the results by July 6, 2020.
• Awarded scholarship funds are placed in the care of the recipient’s approved educational institution by approximately August 1, to be credited to the recipient’s tuition and/or fees in the 2020-2021 academic year. Any unused funds are to be returned to the IHCA Foundation.

Juckette Family Veteran Scholarship Application Form

This $2,000 scholarship, sponsored by the Linda Juckette Family Gift Fund, is for those who have served in the U.S. military, National Guard or Reserves and are pursuing CNA , LPN, RN or Certified Dietary Manager (CDM) programs in Iowa, with the goal of working in long-term care. Four of these scholarships will be offered.

  • I certify with my initials below that I meet all of the requirements for a student in good standing at an approved educational institution. I certify that all of the information contained herein is true and correct.

  • Required Documents

  • Compose and upload an essay of up to 300 words. It must be your own work and include discussion on the following: 1) Why you are interested in long-term care, 2) Any previous health-care related experience, 3) Your educational goals related to long-term care and 4) Your career plans once your education goal is achieved.
  • Applicants currently enrolled in a college must submit a copy of their most recent transcript. (A transcript printed from the college's offical website is acceptable.) Applicants who are newly enrolled students (without an established college transcript) must submit a copy of their program acceptance letter from the college.
  • Upload a recommendation letter from your military supervisor or superior. Please ask them to include your name, their name, organization name, telephone and email address in the letter. In the letter, they should describe why they believe you would be a worthy recipient of an IHCA Foundation scholarship. Please limit letters to 200 words or less.
  • Upload copy of veteran’s record of service demonstrating an honorable discharge or letter from commanding officer certifying active duty status or copy of duty orders.
