I-SNPs and ACOs: Are They Right for Your Organization?, Nisha Hammel
As the healthcare landscape shifts deeper into managed care and value-based arrangements, it is important for long-term care providers to better understand the options available for succeeding in this environment. Join this session to learn of the primary LTC population health models (I-SNPs and LTC focused ACOs), discuss the successes and challenges, and identify the most important reasons to consider these models. 

The Evolution of Value-Based Purchasing in Long-Term Care, Fred Bentley
States have embraced value-based purchasing (VBP) models to capture more value for the dollars they spend on Medicaid long-term care services. In this session, ATI Advisory’s Fred Bentley will highlight the key elements of effective VBP models, with a focus on the key performance measures, performance assessment methodologies, and incentive structures that are yielding positive results. This session will conclude with strategic recommendations for Iowa’s LTC providers to capitalize on the opportunities that a VBP model will create in the coming years. 

Medicaid Reimbursement and Case-Mix Update, Jeff Steggerda and Daleen Seibold
SNF providers know that 2024 is a rebasing year and the first time that Case-Mix Indexes using PDPM will be uses for rate setting.  The presentation will discuss at a high level the key factors that will impact the rebasing calculations using 2024 Medicaid cost reports.  The materials will include sample rate calc’s with some estimates to help providers plan for 2025 and 2026.  PDPM case-mix scoring is new to Iowa and is significantly different than RUGS.  Providers have received initial PDPM reports and the presentation will include discussion of how providers can use this info.  Additionally, the speakers will offer, at a high level, the key monitoring areas to optimize CMI and the timing of future rate adjustments.

Off-Cycle Mandatory Revalidations: What SNF Providers Need to Know, Katie Cownie and Adam Freed
This session will walk skilled nursing facility providers through the nuts and bolts of the new required off-cycle revalidations. Discussion will include information about the enhanced disclosures related to management and ownership.

Speaker Bios

Nisha Hammel is the Vice President of Reimbursement Policy and Population Health for the American Health Care Association and National Center for Assisted Living. Nisha leads the association’s policy and strategic initiatives on value-based care and innovative payment including provider-led special needs plans, ACOs, emerging models, and managed care. Nisha is a recognized thought leader with over 20 years of senior leadership and operational experience in post-acute care and senior living. She is a regular speaker at national and state conferences on value-based care and population health. Nisha earned a master’s degree in social work from the University of Michigan and a bachelor’s in psychology from the University of Notre Dame.

Fred Bentley serves as Managing Director for ATI Advisory’s Post-Acute & Long-Term Care practice. He provides strategic consulting services to leading post-acute and long-term care organizations along with health plans, investors, and innovators in the senior care industry. He focuses on payment and delivery system innovations that lead to more efficient, better-integrated care for vulnerable patient populations. Bentley’s work sits at the intersection of payers and providers, helping forge innovative partnerships and new business models that improve the quality, efficiency, and patient experience for Medicare and Medicaid beneficiaries. As a keynote speaker and panelist at industry conferences, he is frequently asked to provide an expert perspective on the future of the post-acute care industry and opportunities for innovation and disruption. Bentley earned masters’ degrees in Public Health and Public Policy from the University of California, Berkeley, and a bachelor’s degree in Political Science from Haverford College in Pennsylvania.

Jeff Steggerda is a Consultant and Co-Founder of Brighton Consulting Group.  Jeff has worked in the long-term care profession since 1986 and he began working with IHCA in 1993 as a Reimbursement Consultant.  His roles for IHCA have included creating and maintaining several informational databases to support the quality, finance, and data objectives of the association.  His career path includes public accounting and positions as Director of Accounting with regional long term care companies.

Daleen Siebold is IHCA’s Professional Consulting expert and an MDS certified clinical reimbursement nurse with nearly 30 years of long-term care and MDS experience and a solid reputation for providing sound guidance and advice on the MDS process. She has overseen five different buildings as a regional nurse consultant, served as a senior director of clinical reimbursement, as well as a director of nursing and MDS/Medicare coordinator. IHCA members have access to Daleen’s expertise in MDS, Medicare guidelines, PointClickCare, assessments and care planning through IHCA’s Professional Consulting service.

Katie Cownie is a Partner with Dorsey & Whitney LLP.  Katie has been practicing in the health care transaction and regulation space for the last 25 years.  Katie assists skilled nursing facilities and other long term care providers, hospitals, health systems, dental practices, home health agencies, mental health providers and other health care practitioners. Katie has experience representing clients in Certificate of Need related matters, in matters with the Iowa Department of Inspections, Appeals, and Licensing, and in matters with Iowa Medicaid.  Additionally, her transactional practice spans contracting, joint ventures, mergers, and acquisitions.

Adam Freed is a Partner with Dorsey & Whitney, LLP.  Adam represents a wide range of health care providers, including nursing facilities, assisted living programs, home health agencies, hospitals, physician groups, and individual practitioners. For facilities undergoing changes of ownership, Adam advises clients on issues related to licensing, Medicare and Medicaid enrollment, and credentialing with Medicaid managed care organizations and commercial insurers. Adam also advises clients regarding compliance with the State of Iowa’s Certificate of Need (CON) program and routinely appears before the State Health Facilities Council.
