Care Coordinator

2nd shift: Full-time opportunity available

DUTIES: Provides outstanding client care under the supervision of the atHome staff, according to daily assignment sheets as well as advocating for changes and responses as seen appropriate to meet clients’ needs. Promptly responds to client needs and takes appropriate action; serves as primarily coordinator of all services and solutions delivered to assigned client caseload. Schedules in-home employees for daily assignments, resolving related problems such as illnesses, insufficient staffing, client concerns and suggestions, etc.

REQUIREMENTS: Certified in CPR and First Aide, or willing to obtain. High school diploma or equivalent. Prior scheduling experience preferred including developing and maintaining work schedules. Prior healthcare and supervisory experience preferred. Excellent oral and written communication skills. Own transportation required to visit clients. Must have a valid Iowa Driver’s License.
