Dementia Nurse
Responsibilities include but are not limited to:
• Adaptable and proactive to change, the Dementia Nurse accurately completes daily assignments, charting and administrative duties, while adapting to residents’ evolving needs and providing Person Centered Care.
• Participates in dementia specialized training and development efforts, along with the willingness and desire to share recommendations with the Dementia Lead RN on how to continually improve our memory support efforts.
• Directly influences resident care plans and proactively adjusts, consults and coordinates with health care team members to asses, plan, implement and evaluate resident care plans.
• Serves as both a leader and nursing department team member, providing positive and direct influence over CNA’s and CMA’s to establish and maintain optimum resident care standards.
• Sets high standards of excellence for meeting resident needs, builds and maintains effective relationships, communicating in a genuine manner, using appropriate verbiage, and establishes good rapport with the residents, families, affiliates and team members.
• Assists physicians in rounds and reviews orders. Orders medications, maintains resident chart, receives and documents physician orders. Notifies physician and family of incidents or changes in condition of resident.
• Adheres to all company, federal, state and nursing guidelines, policies and procedures, such as the American Nurse Association’s Scope and Standard of Practice to not only meet but exceed quality of care and nursing standards.
• Upmost confidentiality, privacy and implementation of Resident Rights rules, treating residents with kindness, dignity and respect, while also promptly reporting all resident grievances to management.
• Understands and follows all established Universal Precaution guidelines for safety and infection control, as well as federal, state and organization processes and procedures. Reports hazardous conditions and/or equipment to appropriate persons.
• Effectively communicate a resident’s condition and progress with family, affiliates and team members, via multiple platforms, such as in-person, over the phone and via email.
• Requisitions necessary supplies to meet unit needs, such as sanitation, cleanliness, proper storage of medication and hazardous materials, adhering to safety rules and disaster preparedness procedures, as well as ensure equipment is in good working order.
