LPN Nurse Supervisor (Assisted Living)

Luther Park Community is seeking an LPN Nurse Supervisor to join our team!

The LPN Nurse Supervisor provides nursing care for general assisted living and memory care residents of the Gardens at Luther Park Community and helps supervise the nursing staff. The LPN Nurse Supervisor will be knowledgeable of all state and federal laws, rules and regulations applicable to assisted living. The individual works with the Manager of Health Services to provide a physical, social and psychological environment conducive to maintaining the highest level of independent function and well-being for all residents of the program.

Essential Functions
• Help maintain adequate staffing 24/7 to meet the needs of the residents.
• In charge of putting together the monthly schedule for nursing staff and helping fill shifts.
• Communicate with family members regarding the condition and progress of the resident, including in-person and telephone discussions where appropriate.
• Ensure the facility operates in compliance with all local, state, and federal regulations
• Provide leadership and direction to all nursing staff
• Receive and follow up on faxes and help with completing med changes.
• On call every 3-4 weeks for one week, including the weekend.
• Assist with and submit records and documentation as required by internal policies, state and federal rules, regulations and laws for assisted living facilities. Make sure incident reports are completed on time and followed up on.
• Participate in continuing education to maintain credentials and increase expertise.
• Doing 30 and 90 day assessments that are needed within the needed time.
• Help with monthly change over
• Do monthly MAR audits
• Documentation on each resident of changes or things that are occurring and also with families after conversations are had.
• Being able to be on call when needed and able to address issue that come up after hours when staff reach out to for nursing advise
• Follow up on acute assessments happening daily with the residents

Minimal Requirements
• Possess an active registered LPN license, in compliance with Iowa Board of Nursing requirements and standards.
• Must be able to pass a background check and post offer physical and urine drug screen.
• 1 year of experience with assisted living or long-term care programs is preferred.
• Knowledge of MS Office products including Word, Excel, Outlook.
• Ability to respond to common inquiries or complaints from residents, family members, staff, regulatory agencies or members of the business community.
• Possess initiative, good judgment, and leadership ability to work well with the community, residents, families and staff.
• Possess understanding and compassion for the elderly population.
• Ability to build effective working relationships with people of all backgrounds.
• Excellent verbal and written communication skills.
• Self-directed and ability to work independently.
