LPN with Unity Point: In partnership with atHome with Western Home

Full-time, 1st shift opportunity (32 hours/week)
atHome in partnership with Unity Point

Provide quality in-home care for residents living in the greater Cedar Valley, or within Western Home Communities

DUTIES: Assist in the preparation and planning of new client admissions including assessment and appropriate admission content. Maintain awareness of the health status for all atHome clients. Assist clients in maintaining their medication list and provide referral appropriate care. Maintain accurate and timely documentation of clinical records. Provide for emotional and physical comfort and safety of clients, and emotional support to families. Successful functioning in this position contributes directly to the overall Western Home mission of the best care for clients.

REQUIREMENTS: Currently licensed to practice as an LPN in the State of Iowa and CPR certified. Clerical and leadership skills beneficial but not required for growth and development in the area.
