MDS & Care Plan Coordinator

Requirements for this position include:
– Current RN license
– A minimum of 1 year experience conducting MDSs and CAAs in a SNF
– Experience completing skilled assessments in both Med Surge and Skilled Nursing settings
– Experience writing Care Plans
– Experience with electronic health records; Point Click Care preferred
– Knowledge of clinical and federal regulations for SNFs
– Ability to work independently and/or with a team of outstanding leaders in the field
– Ability to study and learn quickly through online courses, consultants and attending outside education
