MDS Coordinator

As the MDS Coordinator you will lead the all aspects of the resident assessments, care planning, completion of the MDS and RAPS.The MDS Coordinator compiles information and data from health records and personal assessments for use in the evaluation of quality of care and utilization review. You will determine the most appropriate time for admission, quarterly, annual and significant change assessments. As the lead in this area, the MDS Coordinator will ensure all team members are aware of and complete scheduled and unscheduled assessments.
