MDS Coordinator

Winslow House Care Center is seeking a Full-Time MDS Coordinator for our 50-Bed Skilled Nursing Facility. The RN MDS Coordinator conducts and coordinates the development and completion of the Resident Assessment in accordance with the requirements of the Federal and State regulations as well as company policies and procedures.

The MDS Coordinator will assess each Resident’s needs and coordinate with family, interdisciplinary staff, and the Resident’s themselves, to develop an individualized care plan that will enhance each Resident’s independence, dignity, safety, and autonomy.
