Nurse Mentor (Creekside Living)

DUTIES: The clinical mentor will manage and administer nursing services within Creekside Living according to WHC protocol and policies. They will assign, delegate, supervise and evaluate RNs, LPNs, as well as others involved in the household team. The clinical mentor will develop the nursing component of the resident plan of care by assessing, planning, implementing and evaluating the physical condition of the resident when necessary. This individual will participate in the documentation and completion of the MDS. The position requires collaboration with other members of the multidisciplinary team to achieve the optimum level of functioning for the resident. The clinical mentor will also conduct employee discipline, performance planning, counseling sessions and staff meetings as necessary in collaboration with the DON.
REQUIREMENTS: Current RN licensure in the State of Iowa. CPR certification or willing to achieve. Nursing experience in long-term care. Previous experience with MDS, geriatric residents and supervisory training. Must have excellent organizational and time management skills and be able to work under stress when deadlines or critical situations arise. Will be responsible for fulfilling on-call requirements including nights, weekends and holidays as needed. Must be able to read, write and speak fluent English. Maintains a positive, professional demeanor when conducting business within WHC and out in the community.
