PT Personal Service Assistant
Under the supervision of a RN, the Personal Service Assistant assists older people who are
functionally, physically, and/or socially impaired and need 24-hour oversight. The Personal
Service Assistants role is to assist tenants with those activities of daily living that they are unable
to perform without help, fostering at all times tenants’ independence and freedom of choice. All
responsibilities will be conducted in a manner that is consistent with the philosophy of assisted
living. This position is responsible for delivery of quality service, promoting good public relations,
and for upholding the mission and standards of the facility.

DELEGATION OF AUTHORITY
Authority is delegated to the individual in this position to:
-Provide care to tenants following facility policies & procedures.
-Report tenants’ needs and concerns to a licensed nurse.
Position reports to: RN Program Cordinator
PRIMARY FUNCTIONS & RESPONSIBILITES
Each individual Tenants Service Plan identifies specifically what areas they are requesting
assistance in. Below is a breakdown of those areas; additional duties may be added as necessity
arises to met each tenants needs.
ESSENTIAL FUNCTIONS:
Activities of Daily Living
1. Perform activities of daily living per company policies & demonstrated competency.
2. Assist tenants with dressing & undressing (clothing should be appropriate for the season).
3. Assist with the selection of clothing
4. Assist tenant with bathing
5. Assist with personal hygiene (shaving, dental hygiene, nail care, foot care) activities.
6. Assist with mobility devices (cane, walker, wheelchair, crutches) including monitoring of safe
transfer and ambulation techniques.
7. Assist in the care of ADL devices such as eyeglasses, contact lenses, hearing aides
8. To assure resident stafety
Instrumental Activities of Daily Living
1. Provide assistance with telephone (assists with calls, phone repairs, hearing & vision
adaptive devices) activities.
2. Provide assistance with housekeeping tasks such as vacuuming, empting trash, cleaning
bathroom, bed making, and bed change as assigned.
3. Provide assistance with laundry tasks, as assigned.
4. Escort & encourage tenants & coordinate internal transportation needs; i.e. meals/activities.
5. Assist with mail delivery and personal correspondence (if requested).
Health Maintenance & Monitoring Tasks
1. Observe and report changes in tenant’s physical condition and cognitive/emotional status to
RN Program coordinator, as needed.
2. Conduct room checks and resident rounds.
3. Monitor environmental safety hazards.
4. Record incidents, errors, or accidents in accordance with facility policy and federal, state
and local regulations.
5. Record tenant and tenant related events in accordance with facility policy and federal,
state and local regulations.
6. Monitor and record health, behavior and miscellaneous needs of tenants and communicate information with other staff members and management.
Dining
1. Assist in a variety of tasks related to dining (escort tenants to dining room, open condiment
packets, cut up meat, etc.)
2. Serve tenants their meals.
3. Prepare breakfast meals per tenants request.
4. Wait on tenants while they dine (fill water, get coffee, re-heat meal, etc.)
5. Clean up kitchen and dining room area after meals per cleaning schedule.
Activity Program
1. Responsible for participating in and supporting the scheduled weekly activities.
GENERAL FUNCTIONS
1. Observe and abide by all HIPPA rules and regulations to ensure that personal health
information is protected during collection, use, disclosure, storage and destruction with
Prairie View Inn; and to ensure only the minimum necessary information is known to function
in this position
2. Follow written and oral directions.
3. Maintain confidentiality of tenant and facility information.
4. Assist new employees in following established facility policies and procedures.
5. Complete assignments timely, completely and accurately.
6. Attend and participate in all in-service classes as assigned and complete assignments.
7. Participate in interdisciplinary team and other facility meetings as assigned.
8. Consistently work cooperatively with tenants, co-workers, physicians, families, consultant
personnel & ancillary service providers.
9. Observe all facility safety policies & procedures.
10. Is observant of safety hazards and emergency situations, and reports to appropriate
person or takes corrective action according to established procedures.
11. Come to work in a clean, neat uniform and consistently present an appropriate
professional appearance.
12. Come to work as scheduled and consistently demonstrate dependability and punctuality,
complies with attendance policy.
13. Assume accountability for data contained in employee’s handbook.
14. Assume accountability for compliance with Federal, State and other regulations within
scope of control and of which informed.
15. Perform all duties assigned in an effective, timely and professional manner.

2. Perform CPR & First Aide (if trained to do so).
LEADERSHIP
1. Plan and complete duties with minimal direction from supervisor.
2. Relate to tenants, families, staff and community members in a courteous and diplomatic
manner under all circumstances.
3. Follow up with appropriate staff, tenant, or other individuals regarding reported complaints,
problems, and concerns.
4. Be aware and familiar with each individual tenant’s status and needs on a daily basis.
5. Act as ambassador and public relations representative to guests and other visitors.
6. Maintain all certifications required for employment.
ADMINISTARTIVE RESPONSIBILITIES
1. Monitor appropriate use of tenant supplies, office and other need supplies and reports to
Program Director/RN Program Coordinator when quantities are low.
2. Copy special paperwork forms at direction of Program Director/RN Program Coordinator.
3. Be knowledgeable of emergency and evacuation policies, as well as conducting and
documenting practice procedures as assigned.
OTHER DUTIES
1. Demonstrate an appreciation of the heritage, values, and wisdom of the tenants and
understanding of the aging process.
2. Attend and participate in staff meetings.
3. Assist with quality assurance programs when indicated.
4. Participate as team member to ensure that the department workloads and goals are met.
Adapts to changing workload and work environment.
5. Assist in the coordination of; move-out, ensuring labeling & packing of tenant’s belongings.
6. Answer lights promptly and take care of tenants needs in a kind considerate manner.
7. Aware of tenant rights. Treat tenants with consideration, respect and full recognition
of his/her dignity and individuality.
8. Listen to report from the previous shift and assist with any functions that still need to be
performed before the end of the shift.
9. Perform incidental housekeeping and maintenance tasks as may arise during the course
of regular duties, in order to maintain a clean, safe, pleasant environment for residents,
visitors, and staff.
10. Knowledgeable and adheres to standard precautions per facility policy.
