General Job Summary:
The Medicare Coordinator is responsible for all direct resident care activities. Provides direction, supervision and evaluation of all resident care. Implements management systems, facility policies and procedures and coordinates nursing service activities in the Medicare unit. Works cooperatively with facility administration, community agencies, and ancillary service providers to meet the needs of the Medicare unit and its clients. Assumes accountability for nursing service compliance to Federal, State and local regulations.

Duties and Responsibilities:
The following is a list of duties and responsibilities for the Medicare Coordinator. All of the job duties are essential, unless otherwise noted.
 Implements established performance standard of nursing practice and facility policies and procedures.
 Interprets existing policies, procedures and performance standards to personnel, residents, families, community agencies, consultants and ancillary service providers.
 Receives report when coming on duty and takes follow-up action as necessary.
 Conducts resident rounds, identifies problems, and situations that require intervention. Initiates corrective action as necessary. Reports concerns identified and corrective measures to the Director of Nursing.
 Supervises and evaluates the implementation of the “Resident Rights”.
 Implements established nursing service budget guidelines as related to allocation of staff and efficient utilization of supplies and equipment.
 Ensures availability of supplies and equipment for nursing personnel providing direct resident care.
 Conduct counseling/education sessions with nursing personnel as delegated by the Director of Nursing. Reports outcome of all counseling/education sessions to the Director of Nursing.
 Assists Director of Nursing in preparation of employee performance evaluations and makes appropriate recommendations.
 Completes delegated duties completely, accurately, and timely.
 Performs nursing skills, demonstrating knowledge and competence.
 Implements facility admission, discharge, and transfer procedures. Supervises coordination with other departments, physicians, residents, families, and community agencies. Verifies that all related documentation is accurate and complete.
 Assess resident needs daily and develops individualized plan of care. Completes or oversees the RAI process on each client.
 Conducts resident care conference with facility personnel providing direct care.
 Provides direct resident care as necessary and as assigned by the Director of Nursing.
 Organizes time to allow sufficient time for required documentation.
 Demonstrates consistent ability to work cooperatively with facility Administrator, Director of Nursing, nursing personnel, physicians, residents, families, other facility personnel, consultants, community agencies, and ancillary service providers.
 Demonstrates concern for resident’s welfare by rendering immediate assistance as necessary.
 Greets resident by name and listens to resident, family and physician concerns attentively and initiates corrective measures and/or reports concerns to the Director of Nursing and Administrator.
 Controls angry feelings and takes disciplinary action privately.
 Maintain resident information and Fountain West internal affairs in strict confidence.
 Preserve resident dignity by always knocking on resident room doors before entering.
Qualifications:

 Education
o Must be a graduate of an accredited school of nursing and must possess and be responsible for all actions necessary to maintain a current license with the Iowa Board of Nursing. Registered Nurse is preferred, however a Licensed Practical Nurse is acceptable. Advanced preparation in geriatrics preferred. Knowledge of the Medicare regulations and experience on a Medicare unit a plus. Must be able to read, write, and follow oral and written instruction in English.
 Continuous Education
o It is the responsibility of the Medicare Coordinator to meet all continuing education requirements, whether internal or mandated by Iowa Board of Nursing. Failure to maintain a current license with the Iowa Board of Nursing is grounds for termination.
 Work Experience
o Previous supervisory experience preferred.
o Six months experience in caring for skilled/long-term care clients is required.

Physical Activity Requirements:
 Lifting Requirements
o The Medicare Coordinator is frequently called upon to lift up to 10 pounds. The Medicare Coordinator will occasionally be called upon to lift objects up to 50 pounds. Occasionally, the Medicare Coordinator will be required to assist with lifting residents weighing up to and exceeding 160 pounds.
 Carrying Requirements
o The Medicare Coordinator is regularly called upon to carry items up to 10 pounds. The Medicare Coordinator occasionally is called upon to carry objects up to 25 pounds. Carrying in excess of 25 pounds is not required.
 Sight Requirements
o The Medicare Coordinator is required to have 20/20 corrected vision.
 Hearing Requirements
o The Medicare Coordinator is required to have normal range of hearing, as corrected
 Socialization Requirements
o The Medicare Coordinator is required to work with all other persons in the Nursing department-the ability to get along and socialize with others in the department is essential. In addition, the Medicare Coordinator will come into regular contact with residents, families, and employees from other departments. Good communication skills are essential.
 Other Physical Requirements
o The Medicare Coordinator will regularly be required to stand, sit, and walk. The Medicare Coordinator will occasionally be called upon for crawling, squatting, kneeling, bending and crouching.
