Social Services Coordinator

The Social Services Coordinator at Sunrise Terrace ensures that each resident’s social, emotional, and psychological needs are met from admission to discharge. As a resident’s needs change, the Social Worker works closely with the Interdisciplinary Team (IDT) to ensure the resident functions to their full potential.
Under the Supervision of the Administrator the Social Services Coordinator will:
– Advocate for residents and assist them in the assertion of their rights within the facility in accordance with Resident Rights, Freedom from Abuse/Neglect/Exploitation, Transitions of Care, Resident Assessments, and Person-centered Care Planning.
– Participate in the Admissions process by identifying and completing required documentation and assessments
– Work with the IDT to create and continuously update a comprehensive plan of care for each resident
– Meet the needs of residents who are grieving from losses and coping with stressful events
– Identify and promote individualized, non-pharmacological approaches to care that meet the mental and psychosocial needs of each resident
– Assist resident and family members with financial and legal matters; applying for services, Medicaid application process, referral to lawyers, referral to funeral homes for preplanning arrangements, etc.
– Make transitions of care seamless by assisting the resident in finding community placement, completing the application process, arranging for home care services for residents returning home, or assisting with transfer to another facility.
– Provide or arrange for needed mental and psychosocial counseling services
– Oversee PASRR and ensure compliance with all regulatory requirements
– Make referrals and arrange needed services from outside entities, including medical transportation
– Make arrangements for obtaining items such as clothing and personal items
– Prepare assigned Quality Assurance documents and participate in monthly meetings
– Track and provide monthly transfers and discharges to appropriate entities
– Provide education and orientation to new employees
– Serve as the facility’s Grievance Official
– Serve as Administrator’s assistant to manage and track facility programs and documents; facility website, Facebook page, employee phone numbers, emergency preparedness program, etc.
– Other duties as assigned
Qualifications:
– A Bachelor’s Degree in Social Work or a Bachelor’s Degree in a human services field, including, but not limited to, Sociology, Gerontology, Special Education, Rehabilitation Counseling, and Psychology
– One year of supervised social work experience in a health care setting is preferred
– CPR required upon hire
