CMS has released new survey resources to support the national implementation of the Risk-Based Survey (RBS) process for nursing homes. Beginning Sept. 8, CMS will implement this new approach nationwide, marking one of the most significant changes to the nursing home survey process in nearly a decade. Facilities should not view the RBS as a shortcut or reason to reduce survey readiness efforts. Instead, CMS's approach reinforces the importance of maintaining strong systems of quality assurance, staffing stability, regulatory compliance and accurate reporting year-round.

CMS recently finalized the removal of the Healthcare Personnel (HCP) COVID-19 Vaccination Measure from the Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) in the Fiscal Year (FY) 2027 SNF Prospective Payment System (PPS) Final Rule. As a result, nursing homes are no longer required to report calendar year (CY) 2026 HCP COVID-19 Vaccination Measure data for the FY 2028 payment determination. SNFs that do not report this data will not be penalized through the FY 2028 annual payment update under the SNF QRP. Make sure staff responsible for National Healthcare Safety Network (NHSN) reporting understand that only the HCP COVID-19 Vaccination Measure has been removed from the SNF QRP, but other required NHSN reporting continues.

CMS continues to prepare nursing homes and vendors for the transition of Payroll-Based Journal (PBJ) submissions and reporting from the current Quality Improvement and Evaluation System (QIES) to the Internet Quality Improvement and Evaluation System (iQIES). The August 2026 Performance Score Reports (PSRs) for the fiscal year (FY) 2027 Skilled Nursing Facility (SNF) Value-Based Purchasing (VBP) Program are now available to download via the Internet Quality Improvement and Evaluation System (iQIES). The August 2026 PSRs provide SNFs their performance information for the FY 2027 SNF VBP Program year, including the incentive payment multiplier. The FY 2027 SNF VBP Program year is the first to assess performance on eight quality measures rather than four measures, as part of an expansion of the SNF VBP Program. CMS recently posted an update to the Five-Star Quality Rating System Technical User’s Guide, effective with the July 2026 refresh. CMS is implementing an increase to the quality measure (QM) rating thresholds, increasing each rating threshold by one-half of the average improvement in QM scores (as previously outlined in QSO 19-08-NH). While the thresholds used to assign QM star ratings have increased, the point thresholds for the individual QMs have not changed. As a result, some facilities may need higher overall QM performance to maintain the same QM star rating.

Several of Iowa’s licensing boards have recently received reports of scammers calling licensees and posing as board investigators, claiming to have received complaints against that licensee. If a potential scam victim believes a caller could be from a licensing board but is unsure, they should take a name, hang up and call the Iowa Department of Inspections, Appeals and Licensing (DIAL). Anyone who receives a suspicious call should protect their confidential information and not engage with anyone claiming to be an Iowa licensing board staff member until they can confirm via return call that the request is legitimate.

CMS has released FY 2026 guidance for Federal Monitoring Surveys (FMS), outlining how federal surveyors will oversee state-conducted nursing home surveys, including health, Emergency Preparedness (EP) and Life Safety Code (LSC) Surveys, through Sept. 30, 2026. Due to survey disruptions caused by the 2025 federal government shutdown, CMS reduced the required number of monitoring activities by 10%, resulting in two fewer Resource and Support Surveys in Iowa compared to the previous fiscal year. The guidance also details three federal oversight activities – Resource and Support Surveys, Comparative Surveys and EP/LSC Desk Audits – and notes that CMS may take enforcement action if deficiencies requiring correction are identified.

The International Code Council (ICC) recently approved restoring nursing homes to Risk Category III for the 2027 International Building Code (IBC). All nursing homes are required to report on annual influenza vaccination among health care personnel (HCP) by Monday, May 18, at 11:59 p.m. PST to meet CMS guidelines, as required by the Skilled Nursing Facility Quality Reporting Program for the 2025-2026 influenza season. The reporting period for the 2025-2026 influenza season is Oct. 1, 2025, through March 31, 2026. Facilities are only required to submit one report that covers the entire reporting period. Nursing homes that fail to report the required data by the deadline may lose 2% of their Medicare reimbursement for fiscal year 2027.

With the new process, digital certificates will be posted in DIAL’s Health Facilities Database in the provider’s entity detail page in the ‘files’ folder. The Fee for Service (FFS) Advance Beneficiary Notice of Noncoverage (ABN), Form CMS-R-131, has been updated. The updated form is effective now and expires March 2029. Providers may continue to use the expired version of the form until May 12, 2026, but must begin using the approved form no later than that date. This ABN is used in nursing facilities and other provider types to transfer potential financial liability for items/services (such as outpatient therapy) expected to be denied under Medicare Part B only.

CMS recently updated the Long Stay Antipsychotic measure calculation to include claims data and identified some residents with schizophrenia who should have been excluded from the measure. CMS is correcting the calculation and plans to update the measure’s cut points used in Five-Star ratings. The Iowa Department of Inspections, Appeals and Licensing (DIAL) has announced final rule changes to Iowa Administrative Code Chapters 481-67 and 481-69, effective March 11, 2026. The updates revise the scope of practice for LPNs, allowing them to complete initial and change-in-condition assessments by delegation from a Registered Nurse, and require programs to specify in policy who is responsible for completing initial incident reports.
